Document use cases, not only tool names
The same AI tool can be used for very different tasks. Record the purpose, inputs, output and decision point for each use case so the responsible person can understand the actual process.

Update when the process changes
A register becomes misleading if the model, purpose or data changes but the entry does not. Assign a review date and a person who can update or close the record.
